I definitely remember The Sims 2's Elder underwear there! Giving some overdue thanks for that.

Also, is the underwear from The Sims 3 from its base game or not?

Woops! This reply is pretty ancient by now, but I'll answer it anyways  Yep they are! For obvious reasons, it's only available for elders, although they can also wear the default bra's that adults can also wear.
