That's just a picture. Do you have a link to the actual download?

- Put the files (should be two "package" files) in your Downloads folder.
- Go ingame, find the box (looks like the little square thumbnail down to the right - poseboxes are usually found in either Hobbies/misc or general/misc)
- Choose a sim, click on the box, choose a pose. The sim should snap into the pose.
- Choose another sim, click on the box, find the pose that matches (the overview picture should be helpful). The sim will snap into position.
- Move the sims together using the cheats "Moveobjects On" and "Boolprop snapobjectstogrid false" (you may also need OMSPs, I recommend the black one in this set). You can also use the cheat "boolProp allow45DegreeAngleOfRotation true" for a 45 degree angle option.
- Take a picture.

The numbers over the poses refer to the Stretchskeleton cheat, which you can use on the male sim to make the poses go together better (you'd select a sim, then write for instance Stretchskeleton 1.13 in the cheat box - you can reset it with Stretchskeleton 1.0)

This video shows how you'd go about ingame: https://www.youtube.com/watch?v=x2ZRQiBimZM
