What is this 'who?' that's showing up besides agree/disagree/helpful/funny/love for? Is it to try and spot who disagreed with you?

Is there a full on, complete tutorial on how to literally extract ingame objects and use them as regular ones? I really want to try and make some accessory items into deco myself, but haven't been able to find a good enough tutorial (The only one I've found is ancient). I know there are already extracted objects, just want to try my hand at doing it myself.

They gain or lose recovery points while off-lot, but the calculation isn't actually applied until they come home. What can then happen is that they return home and immediately die next to the mailbox. It is risky to send a sick sim to work if you have the Real Sickness mod in.
I should think an ancient tutorial is likely sufficient? Not much will have changed in the meantime.

For the actual "make a deco object" part, you need a start-to-finish tutorial on how to make an object (if you don't know how to already).
For extracting, you can use the "Finder" tab with "Namemap" selected (in SimPE), then search for a word that matches the item you're looking for (if you're unsure exactly what it's called). If you find it, double-click the GMDC, and it pops up in the Plugin View. Extract (right-click/extract to get the GMDC/SMD, or click the Extract button to get the OBJ file). Go back to the Finder, double click the LIFO and/or the TXMTs to get the textures ("Extract"). The LIFO with 0 in it is the largest, not every item has a LIFO (LIFOs are the best quality/largest versions of the textures).
You can also check out the files in the "\TSData\Res\3D" for the individual pack, if you know which pack the item comes with (the "Finder" searches through the packs).
Depending on the item, you may have to move/rotate it to make it look like deco, as some acessory objects have an unusual rotation.
But really, most of what you need besides the tutorial is to extract a texture and a mesh.

Cloning an item also clones the properties of that item, which usually means you'd have to fix a bunch of other things.
If you want to make a deco object, the best thing is to clone a deco object that's as close as possible to the item you want in properties, and replace the mesh and texture (which you can extract from the game - see above).

Sims at work are not off-lot. They are still on the lot, they are just in the off-world room.

What would be considered as "off-lot," then? Because in that case I imagine that anything that adds the errands icon next to the Sim portrait acts the same way (hovering over the Sim stack on the left hand side and reading Name Surname (At Work/At School/On a Hike/Snuck Out/Running Errands)). Except for maybe being abducted?

Generally, only non-household members can truly be off-lot. Probably Pescado was just referring to how diseases don't progress on community lots.

Speaking of off-world Sims, does anyone know what happens if a social worker shows up while children are at school? I'll probably try it in a test hood later to see for myself.

Please test it out and tell us! If I remember correctly, the social worker spares the children that are not home at the moment and doesn't take them. I think I've had that happen, but maybe it was a fever dream??? I don't remember.
Here's a side note: I one time had a single mom with four children, and I made them walk to a community lot. I think some of the children disappeared first, then the mom, and one or two were just behind her. As soon as the mom vanished, I got the "children shouldn't be left alone" message, but I was redirected to the community lot. Over there, it was just the mom and 3 of the children. When they went back home, the social worker was gone, but so was one of the kids. So... in a way, she only took one and spared the other three because they were "not home".
